Activating Support for Self-Management, London – 23 November

This seminar will discuss how commissioners and clinicians can engage with self-management with a worked example from Oxfordshire PCT, which has pioneered a different model of long-term condition service provision in partnership with the Expert Patients Programme Community Interest Company (EPP CIC).

EPP CIC is working with commissioning groups across the country to introduce a new initiative which puts patients at the centre of their healthcare, improves the lives of people with long-term conditions and produces genuine savings in primary and secondary care costs. ‘Self-Management for Life’ provides an integrated approach which trains both healthcare professionals and patients so that patients are activated to self-manage effectively in partnership with their clinician.

This event is aimed at individuals from Primary Care Trusts, GP Consortia, Public Health bodies, Local Authorities, the Department of Health and other organisations responsible for services to people with long-term conditions.
